Locations

The locations of each home allow for privacy and convenience, quiet residential neighborhoods in close proximity to local parks as well as major shopping centers and regional healthcare services all within minutes of three major metro area highways.  Each home has been thoughtfully modified to meet the accessibility needs of individuals with mobility issues while carefully blending into their local neighborhoods. Each home is meticulously maintained to complement the community and to maintain the dignity for all individuals living in a community residential setting.  Depending on the needs and/or personal desire of individuals in need of supports the homes are set up for communal living with the majority of the home outside of private bedrooms being common area or the home has been modified with independence in mind; allowing each individual a 'suite' to call their own including a bedroom, bathroom (accessible) and living room while still maintaining the security of 24 hour shared support staff onsite.

Our Team

Our team of Direct Care Staff provide caring hands on support to the individuals living in the home while ensuring that they along with the individual(s) served are a valued part of their local communities.

Our Program Managers work in and support the day to day goings on of each home, the Program Manager along with a Program Director are responsible for ensuring the individuals needs are being met and the direct care staff have the training and support they need.

Our team of Program Managers support one home in a team of five homes underneath one Program Director, working tirelessly to ensure that our direct care staff can deliver the very best quality of care while maintaining personal choice and dignity of the individuals served.

All of our teams are supported by our Administrator who along with our Office Staff, Human Resources and Maintenance Department ensure every level of our organization has the tools and resources to meet the needs of the individuals served.

Care Services

Our team of staff are able to meet the physical medical needs of an individual not requiring 24 hour nurse supervision, although we have nurses on staff they are not available onsite 24/7.
Our staff are trained to work with individuals living with significant mental illness or who have a behavioral history as well however we do not provide supports to individuals who pose a serious risk of harm to themselves or others.
Our nursing staff provide training and support for direct care staff as well as professional care management for individuals served. Continuity of care is vital to the success of individuals served and the dedication to this cause by many staff in our organization is evidenced by the number of staff nearing or having exceeded 10 years of service to the individuals served by our organization.
